Santa Rosa Community Health Centers Data Breach – Investigated by Federman & Sherwood
January 13, 2026–Federman & Sherwood is investigating a data breach involving Santa Rosa Community Health Centers, following a notification filed with the Attorney General of California.
According to the notice, the incident stemmed from a cybersecurity breach at TriZetto Provider Solutions, a third-party vendor that works with the organization’s electronic medical record system. Unauthorized access to one of TriZetto’s systems was reported in late 2025.
The information potentially involved includes names, Social Security numbers, dates of birth, contact information, and certain health-related or insurance information. Notification and identity protection services are being coordinated through TriZetto and its vendors.
Federman & Sherwood is investigating whether appropriate vendor oversight and data protection measures were in place and whether affected individuals may have legal claims.
The compromised information may have included:
