Petaluma Health Center Data Breach – Investigated by Federman & Sherwood
January 23, 2026– Federman & Sherwood is investigating a data breach involving Petaluma Health Center, following a notification filed with the California Attorney General.
According to the notice, the breach stemmed from a cybersecurity incident at TriZetto Provider Solutions, a third-party clearinghouse vendor used by OCHIN, which supports Petaluma Health Center’s electronic health record system. An unauthorized actor reportedly accessed one of TriZetto’s systems beginning in November 2024.
The information potentially involved includes individuals’ names, Social Security numbers, dates of birth, addresses, contact information, and health insurance or other health-related information. Petaluma Health Center reported that there is no indication that financial account or payment card information was involved and that identity monitoring and protection services are being made available.
Federman & Sherwood is investigating whether reasonable cybersecurity safeguards and vendor oversight measures were implemented and whether affected individuals may have legal claims arising from the incident.
The compromised information may have included:
- Full name
- Social Security number
- Date of birth
- Contact information (phone number, address, email address)
- Medical information
- Health insurance information
- Other sensitive information
