March 4, 2026 – Federman & Sherwood is investigating a data breach involving Couve Healthcare Consulting, LLC d/b/a Evergreen Healthcare Group, following a notification filed with the California Attorney General.
According to the notice, Evergreen Healthcare Group became aware of unauthorized activity within its cloud-based healthcare platform on or about December 3, 2025. The organization initiated an investigation and engaged third-party cybersecurity specialists to determine the nature and scope of the incident. The investigation determined that certain files may have been accessed by an unauthorized party. The company completed its review of affected files and finalized the list of individuals to notify on February 13, 2026.
The information potentially involved includes personal information provided in connection with patient referrals and medical care.
In response to the incident, Evergreen Healthcare Group reports that it implemented additional technical safeguards, enhanced security measures, and updated internal procedures. The company is also offering affected individuals twelve months of complimentary credit monitoring and identity theft protection services through Cyberscout, a TransUnion company.
