Laboratory Services Cooperative (LSC) Data Breach – Investigated by Federman & Sherwood

Oklahoma City, Oklahoma (April 11, 2025) – On October 27, 2024, Laboratory Services Cooperative (LSC) detected suspicious activity within its computer network. Upon identifying the unusual activity, LSC acted swiftly by engaging independent third-party cybersecurity experts to conduct a comprehensive investigation into the nature and scope of the incident. LSC also promptly notified federal law enforcement authorities to ensure appropriate oversight and coordination during the investigation. Through this investigation, it was determined that an unauthorized third party had gained access to specific areas of LSC’s network and was able to access and extract certain files stored on its systems. In response, LSC immediately launched a detailed internal review to assess the extent of the exposure, and the types of information potentially involved.

In February 2025, LSC received the initial findings from its comprehensive data review, which indicated that certain information related to both patients and employees of LSC may have been impacted by the incident. The specific data involved varies from person to person and is based on each individual’s relationship with LSC.

  • Name
  • Social Security number
  • Date of birth
  • Contact information – phone number, address, email address
  • Medical information – date(s) of service, diagnoses, treatment, medical record number, lab results, patient/accession number, provider name, treatment location, and related-care details
  • Health Insurance information – plan name, plan type, insurance companies, and member/group ID numbers
  • Financial information – claim numbers, billing details, bank account details (including bank name, account number, and routing number), billing codes, payment card details, balance details, and similar banking and financial information
  • Other sensitive information
