Horizon Healthcare RCM Data Breach – Investigated by Federman & Sherwood
Oklahoma City, Oklahoma (June 30, 2025) – The law firm of Federman & Sherwood announces that it is investigating a data breach involving Horizon Healthcare RCM, a healthcare organization based in Crown Point, Indiana. The breach was formally reported to the Attorney General of Maine and is known to have impacted at least six Maine residents.
According to the notice filed with the Maine Attorney General’s office, the breach occurred on or around December 27, 2024, but was not discovered until May 20, 2025. Horizon Healthcare RCM described the incident as an internal system breach, which resulted in unauthorized access to sensitive personal information.
The compromised information includes individuals’ names and other personal identifiers in combination with data elements that, if exposed, can increase the risk of identity theft. In response to the breach, Horizon Healthcare RCM began notifying affected individuals by written communication starting on June 27, 2025. The organization is offering 12 months of complimentary credit monitoring services through Kroll to help safeguard affected individuals against potential misuse of their information.
Federman & Sherwood is actively investigating the scope of the breach, the security protocols in place at the time of the incident, and whether sufficient measures were taken to prevent unauthorized access.
