Federman & Sherwood Investigates South West Family Medicine Associates, PA for Data Breach

Oklahoma City, OK (November 8, 2024) – The law firm of Federman & Sherwood has initiated an investigation into South West Family Medicine Associates, PA with respect to their recent data breach. On or about August 7, 2024, South West Family Medicine experienced a network security incident. Upon learning of the issue South West Family Medicine Associates immediately began an investigation into the nature and scope of the incident. After an extensive forensic investigation and internal document review it was determined that the following personal information was accessed: individuals name, Social Security Number, driver’s license number, date of birth, address and zip code, medication information, medical history, lab results, diagnosis/condition information passwords, personal identification numbers, or other access codes, numbers or information used to access financial resources, passport number, mother’s maiden name, unique private key used to authenticate or sign an electronic record, tax identification number or identity protection personal identification number, and/or health insurance policy number or subscriber information.
