SimonMed Imaging Data Breach – Investigated by Federman & Sherwood
Oklahoma City, Oklahoma (October 15, 2025) – The law firm of Federman & Sherwood announces an investigation into a data breach involving SimonMed Imaging (“SimonMed”), one of the largest outpatient medical imaging providers in the United States. According to the data breach notification filed with the Attorney General of Maine, SimonMed experienced a cybersecurity incident that may have compromised personal information belonging to patients and others who received care through its facilities.
SimonMed reported that on January 27, 2025, it was alerted by one of its vendors to a possible security incident. The following day, January 28, 2025, SimonMed identified suspicious activity within its own network and determined that it had been the victim of a criminal cyberattack. The company’s investigation, conducted with the assistance of cybersecurity and privacy professionals, revealed that unauthorized access to SimonMed’s systems occurred between January 21, 2025, and February 5, 2025. During that period, an unauthorized party obtained files containing patient information.
The compromised data may include names and other personal information, and in some cases, medical or health-related details. While SimonMed stated it found no evidence that the information has been misused for fraud or identity theft, the company has offered affected individuals complimentary credit and identity monitoring services through Experian IdentityWorks.
Federman & Sherwood’s investigation seeks to determine whether SimonMed took adequate steps to safeguard sensitive patient and consumer data prior to the breach.

- Full name
- Social Security number
- Other sensitive information
