CareTracker, Inc. d/b/a Amazing Charts (“Amazing Charts” or “AC”) Data Breach – Investigated by Federman & Sherwood
Oklahoma City, Oklahoma (November 18, 2025) – The law firm of Federman & Sherwood announces it is investigating a data breach involving CareTracker, Inc. d/b/a Amazing Charts (“Amazing Charts” or “AC”), a medical records service provider, following its data breach notification and written notices sent to impacted individuals.
According to the notification, on or about June 19, 2025, Amazing Charts identified unusual activity affecting certain systems operated by one of its third-party service providers. The company immediately secured the impacted environment and retained external legal counsel and forensic cybersecurity specialists to investigate. The investigation determined that there had been unauthorized access to the third-party provider’s systems between June 15, 2025, and June 19, 2025, and that certain files and data relating to patients of Genaro C. Fernandez, MD may have been involved.
The information that may have been impacted includes patients’ names along with certain types of medical information, such as treatment or diagnosis information, physician names, medical record numbers, and/or health insurance information associated with services received from Genaro C. Fernandez, MD. Although Amazing Charts has reported that it has not identified any instances of identity theft or fraud, it is notifying affected individuals out of an abundance of caution.
To assist impacted individuals, Amazing Charts is offering 12 months of complimentary credit monitoring and identity protection services through Experian IdentityWorks. These services include credit monitoring, identity restoration assistance, and identity theft insurance. Impacted individuals have been encouraged to enroll in these services and to monitor their financial accounts and credit reports for unusual activity.
Amazing Charts reports that it is enhancing its security measures and working closely with its third-party service providers to strengthen safeguards and help prevent similar incidents in the future.
Federman & Sherwood is investigating whether CareTracker, Inc. d/b/a Amazing Charts and its third-party service providers implemented reasonable and adequate safeguards to protect the personal and medical information in their possession and whether impacted individuals may be entitled to compensation, extended identity monitoring, or other legal remedies.
The compromised information may have included:
- Full name
- Social Security number
- Medical information
- Health insurance information
- Financial information
- Other sensitive information
