Outcomes One Inc. Data Breach – Investigated by Federman & Sherwood
Oklahoma City, Oklahoma (October 24, 2025) – The law firm of Federman & Sherwood announces that it is investigating a recent data breach involving Outcomes One Inc. (“Outcomes”), a healthcare services provider that partners with health plans across the United States to deliver medication therapy management and adherence programs.
According to a notification filed with the Attorney General of California, Outcomes discovered that on July 1, 2025, an employee’s email account was accessed by an unauthorized third party following a phishing incident. The employee promptly reported suspicious activity, and Outcomes immediately secured the affected account and engaged third-party cybersecurity experts to assist with the investigation.
The investigation determined that, during approximately one hour of unauthorized access, emails and attachments containing sensitive information were viewed. On July 17, 2025, Outcomes confirmed that the compromised data included individuals’ names and one or more of the following: dates of birth, gender, phone numbers, health insurance information, service dates, healthcare IDs, diagnoses, or medication details.
Outcomes reports that it has implemented enhanced security safeguards and strengthened employee phishing-prevention training to reduce the risk of similar incidents in the future.
Federman & Sherwood’s investigation will focus on whether Outcomes One Inc. took adequate measures to secure patient information and comply with federal and state privacy laws, including the Health Insurance Portability and Accountability Act (HIPAA).
